Exclusive to Affinity, the Base Plan is our entry-level offering with 600 lifetime hours of care. This plan provides basic coverage at the most affordable price point, ideal for those with minimal care needs or as a supplementary option to other insurance coverage

* The value of the “Lifetime Membership Hour” for each plan is determined from a current national retail average of $25 to $40 per hour (based on geographic location) for non-medical home care agencies in our network. All plan hours are divided into ten equal segments separated by 90-day reset periods that total the Lifetime Membership Hours of a chosen level contract. The hour in each segments can be utilized as quickly or slowly as desired. Multiple segments can be accessed in a single contract year within the parameters of the plan. Benefit care are limited to $150.00 a day.

 

Note: Members are not permitted to upgrade their plans at any time. However, the option to downgrade is available, allowing members to adjust their coverage to better suit their changing needs or preferences.

Suitability Standard Survey

The Suitability Standard Survey is a crucial step in ensuring that Affinity by True Freedom aligns with the needs and capabilities of our prospective members. While we do not conduct medical underwriting, and our program is not an insurance plan but a service contract, it’s essential that our prospective members are self-sufficient during enrollment. The Suitability Standard Survey is to verify that our program is the right fit for individuals who, at the time of enrollment, are living independently and are capable of self-care without any assistance from anyone. At the time of enrollment, prospective members must meet the following suitability standards:

Independent Living

Are you able to live independently at the time of enrollment, without needing any assistance, with getting groceries, cooking, bathing, and getting to and from places?

Self-Care Capability

Can you perform daily activities such as bathing, dressing, and transferring without assistance from anyone, including family members?

Understanding of Our Services

Are you aware that the True Freedom Homecare Plans are not insurance but service contracts designed to complement your independent lifestyle?

If you answer “NO” to any of these questions, we regret to inform you that you may not qualify for enrollment. Our goal is to provide a solution for individuals living independently, planning ahead for their future home care needs.

  • Independent Living: Our program is tailored for individuals who are currently capable of managing their daily routines independently without assistance. This criterion ensures that our resources are allocated to those who will benefit from our services in enhancing their quality of life while maintaining their independence.
  • Self-Care Capability: The ability to perform daily self-care tasks is a fundamental aspect of living independently. Our program is structured to support those who, despite potentially facing future healthcare needs, do not currently require assistance with personal care. This distinction ensures that our members receive the support they need when they need it, without duplicating services they can currently manage on their own.
  • Understanding of Our Services: Acknowledging that Affinity by True Freedom offers service contracts, not insurance, is vital. This recognition ensures that members have a clear understanding of the nature of our services and how they complement an independent lifestyle. Our program is designed to offer peace of mind and support planning for future care needs, not to provide immediate home care or replace existing insurance coverage.
