How to Use Our Enrollment Forms
We’ve been updating our forms to ensure a smooth and easy process for you. To confirm you have the latest version, check the top right corner of the form for the version date and ensure it matches the date below upon download.
Form Options:
- Member Enrollment Form: A PDF form designed specifically for individual member enrollment.
- Member & Spouse Enrollment Form: A PDF form for couples, including an additional page for Spouse Information.
Instructions for Completing the Forms:
Member Information:
- Purpose: To ensure the member qualifies for our program.
- Action Required: The member must answer “YES” to qualify.
- Representative Information: If you wish to receive recognition, ensure your information is accurately filled out; Rep Name, Rep Email, Group Name, and Rep ID.
Spouse Information (Member & Spouse Form Only):
- Purpose: To ensure the spouse qualifies for our program.
- Action Required: The spouse must also answer “YES” to qualify. Complete all fields with the spouse’s information, ensuring accuracy and completeness.
Terms & Conditions:
- Purpose: To clarify how our program, plans, and services work.
- Action Required: Ensure both you and the member (and spouse, if applicable) read and understand these terms.
Statement of Understanding:
- Purpose: To confirm the member’s (and spouse’s, if applicable) agreement to become members and their understanding of each statement provided.
- Action Required: The member (and spouse, if applicable) must read and agree to all statements.
Representative Responsibility & Acknowledgement Agreement:
- Purpose: To outline the representative’s responsibility in explaining our program, plans, and services.
- Action Required: As a representative, ensure you have provided accurate and complete information about our offerings. This section confirms your commitment to delivering the correct information without altering our messaging.
Discount Information for Member & Spouse Form:
- When enrolled together, both the member and spouse will receive a 10% discount, as indicated in the Membership Plans section.
- Important Note: If one partner cancels their membership, the spouse/partner discount no longer applies. However, if a spouse/partner is deceased, the surviving member will continue to receive their discount, honoring their continued membership.
